What is the Patient Portal?

The Patient Portal, called “My Chart”, is a free secure online website where you can see your health information instantly 24 hours a day.

What can I do on the Patient Portal?

· View your personal health information including your medication list

· Update your contact information

· See upcoming appointments


What do I do if I forgot my password?

1.  If you forget your password,  you can create a new password by clicking on “I forgot my password”. Then enter your email address or User ID and click “Reset Password”.

 2.  Answer your password recovery questions. Make sure you enter them exactly how you originally entered them. Then click “Reset Password”.

3.  Check your email account that you use for the portal and look for an email from the North Colorado Health Alliance. If you don’t see the email in your Inbox, wait a minute and also check your Spam Folder. Insert the temporary password given in the email and click “I forgot my password”.

4.  Create a new password. Remember, the password must contain a combination of letters and numbers, and be between 6 and 15 characters long. Then click “Submit” and log-in with your new password.

How do I register for the Patient Portal?

1.      To register for the Patient Portal, you must have an active email account. You must also ask the front desk clinic staff for your PIN number.


·         Once you get your PIN number, it will be active for 30 days.

·         If you wait to register online and verify your identity after 30 days, you will need to ask for a new PIN number.

·         Make sure you keep your PIN number private and only share it with people who you want to see your health information.

If you are experiencing issues such as accessing your account or resetting your password, please email support.ptportal@nocoha.org. You will receive a response within 48 hours Monday-Friday.
